Indications/Uses
20-mg gastro-resistant tab: Complaints eg, heartburn, acid reflux & pain during swallowing; inflammation of the oesophagus; prevention of gastroduodenal ulcers during long-term treatment w/ NSAIDs. 40-mg gastro-resistant tab: Moderate to severe inflammation of the oesophagus caused by acid reflux; gastric ulcer; duodenal ulcer; gastric or duodenal ulcer caused by H. pylori; Zollinger-Ellison syndrome & other disorders causing an increased production of gastric acid that requires long-term treatment.
Dosage/Direction for Use
20 mg Complaints eg, heartburn, acid reflux & pain during swallowing as a result of reflux of gastric acid to the oesophagus One tab daily for 4 wk. Long-term treatment & prevention of recurrence of symptoms in the event of mild inflammation of the oesophagus One tab daily. Prevention of GI ulcers during long-term use of NSAIDs One tab daily. 40 mg Moderate to severe inflammation of oesophagus caused by acid reflux, gastric & duodenal ulcers One tab daily. Eradication of H. pylori bacterium One tab bd w/ amoxicillin 1 g bd + clarithromycin 500 mg bd; or clarithromycin 250-500 mg bd + metronidazole 400-500 mg bd; or amoxicillin 1 g bd + metronidazole 400-500 mg bd. Zollinger-Ellison syndrome & other disorders involving severely increased acid production Initially 2 tab once daily. If more than 2 tab daily is required, doses should be taken in 2 doses distributed over the day.
Administration
Should be taken on an empty stomach: Take 1 hr before meals. Swallow whole w/ a glass of water, do not chew/crush.
Contraindications
Hypersensitivity to pantoprazole or to medicines containing other PPIs.
Special Precautions
Patients w/ severe liver problems; reduced body stores or risk factors for reduced vit B12 requiring long-term treatment w/ pantoprazole; concomitant use of medicines containing atazanavir; osteoporosis or concomitant corticosteroids use; history of skin reaction after previous treatment w/ PPIs; appearance of rash especially in areas exposed to sun; upcoming Chromogranin A test. May lead to reduced absorption of vit B12. Increased risk of fractures of the hip, wrist or spine in patients taking PPI for a long period of time (>1 yr). Risk of low Mg level in long-term treatment (>3 mth). Associated w/ small increase in infectious diarrhoea. May alleviate symptoms of cancer & may delay its diagnosis. Keep patient on long-term treatment (>1 yr) under regular surveillance. Pregnancy & lactation. Not recommended in childn <12 yr. 20-mg gastro-resistant tab: Concomitant use w/ NSAIDs.
Adverse Reactions
Benign polyps in the stomach.
Drug Interactions
Potential interaction w/ ketoconazole, itraconazole, posaconazole or erlotinib; phenprocoumon & warfarin; atazanavir & other medicines used to treat HIV; fluvoxamine; rifampicin; St. John's wort. Concomitant use of PPIs w/ MTX may elevate & prolong serum levels of MTX &/or its metabolite, possibly leading to MTX toxicities.
